I give permission for photos or videos of this member to be taken and used in Bridgetown Theatre printed and/or online publicity.
By submitting this form and appropriate fee i apply to become a member of the Bridgetown Repertory Club inc. for the current year. As a member, i agree to abide by the aims and rules of the code of conduct*
Membership runs from 1st January to 31st December each year
